Project Funding Application Form
Setting up a new project
Please answer all the questions as fully as you can. This will give us an idea of your project, how it will be run and its aims, as well as help you estimate the resources and budget required. This form may take a few hours to complete but will help you clarify your projectÕs requirements. At present we are seeking sponsorship on a project-by-project basis. The more accurate a picture of your project you are able to give us, the easier it will be for us to put your case to potential sponsors.

Please complete this form electronically or by hand, whichever you prefer. If you would like advice or help at any stage, please ring or email us on the numbers and e-mail address given above.

If you complete the form by hand, please make a copy before posting it to us in case the original goes astray.

Please e-mail or post the completed form to Musequality at the address above. Thank you.

Section B - Your project plan
To plan your project effectively, as well as to assess your financial requirements, it may be useful to break down the process of establishing your project into phases. We suggest the following:

Phase one: Plan your project. Establish the scope of what you will offer, define the target group it will involve, secure any licences or permissions you may require from the relevant authorities, and ensure that you know your legal responsibilities and tax liabilities. You should also identify your key staff members or make sure it is possible to recruit the necessary staff at a reasonable cost. Musequality is unable to offer financial assistance with this stage.

Phase two: Find and prepare a venue. If extensive renovations are likely to be will be required, allow time for this. Be aware that building work is often subject to delays and cost increases and take this into account.

Phase three: Recruit staff. You may need to involve a key staff member to help with recruitment.

Phase four: Purchase equipment. You may need advice from your staff on what is needed. We need to
know how much all the instruments and equipment will cost. We may be able to source instruments for you
through donations.

Section I - Budget for start-up costs
All instruments and equipment purchased or funded by us will remain the property of Musequality for a period of five years or during the funding period, after which ownership may be transferred to the project. In the event of the project closing during that five-year period, we will advise you either (a) to sell the instruments and equipment at market value and return the sum raised from the sale to Musequality, or (b) to transfer them to another project.
During this period the instruments and equipment may not be transferred to private ownership, except by agreement of Musequality staff or trustees, and should not be rented out unless there is a direct benefit to the project. All children requiring instruments or equipment must be given access to them at no cost. An annual stocktake will be required in which a report of the state of the instruments should be given before further funding will be released.

Section K - Funding
We fund projects for up to three years. It is important that you begin now to think about how you will secure ongoing funding for your programme after our support ends. For this reason it is vital that, from the outset, you keep accurate attendance records and record other data that can be used to demonstrate your project's effectiveness. The more successful and efficiently run your project, the better are your chances of securing ongoing funding.

In the preparatory stage, we will require assurances that each stage has been completed before releasing funding for the next phase. Thereafter we will require six-monthly reports on the project giving details of activities and spending.
Are you prepared to provide reports? Yes / No
Section L - Acknowledging MusequalityÕs support
We would like your staff, pupils, audiences and others to know how Musequality has helped your project; equally we would like MusequalityÕs donors to know where their donations have been spent. We will supply notices (which can be printed locally) which should be displayed on or in Musequality project buildings/spaces throughout the term of our sponsorship. We will supply logos to be included on websites and which should be printed on programmes throughout the term of our sponsorship. Where we have been instrumental in establishing a project, but if our sponsorship term has ended or if the project is funded by another organisation or organisations, we would expect our role to be acknowledged in buildings/spaces and on websites and printed materials about the project. Similarly, we expect organisations with which we collaborate to acknowledge our role in their projects on their own websites and literature about that project. We hope, when funds allow, to provide t-shirts for Musequality project students to wear while taking part in Musequality projects, knowing that this enhances a sense of belonging, pride and
achievement.
Are there any reasons why you will not be able to acknowledge our support in the ways we suggest? If so, please give details.

Will you be able to acknowledge our support on:
- your premises (outside, indoors, in performance spaces)? Yes / No
- your website? Yes / No

If so, do you have a local supplier who could produce them at reasonable cost (if we supply the artwork and secure the funding)? Yes / No
